Creating a form that others can fill-out electronically is useful for a variety of purposes. Unfortunately, the tools for creating forms in Word are buried. Follow these steps:

First, we have to enable the menu that contains forms tools, the Developer menu. Click File, then Options.

Click Customize Ribbon on the left side of the window.

Check the Developer check-box and then click OK.

Next, type into your document the name of the first form field you with to create. For example, a First Name field. Insert a few spaces after the field name.

To insert a form field, from the Developer menu, select the Plain Text Content Control (pictured below):

The area where is says "Click here to enter text" is your first form field. Next, let's restrict access to individuals who view this document to keep them from editing anything other than the form fields. Click Restrict Editing.

On the pane that appears, check Allow this type of editing in the document: and select Filling in forms from the drop-down.

Click Yes, Start Enforcing Protection.

Enter a password for protection in the dialog box that appears and click OK.

You're finished! This same process can be repeated to create a full form with various input options. When a user finishes editing the document, their changes can be saved and emailed back to you.
